End date of ACO REACH is December 31, 2026, after which the CMS LEAD model will operate beginning from January 1, 2027, setting a benchmark for 10-year periods. The choices of population health management software vendors today will shape the results of an ACO until 2036. In what follows, it will be shown which software platforms work well with CMS programs and advice regarding build vs buy options.
The Accountable Care Organization is made up of physicians, hospitals, and other providers who are all together responsible for managing the quality and costs of healthcare for a given population. When an ACO keeps costs down and meets quality goals set by CMS, then it shares some of the savings that are generated. Depending on the model, the ACO can also share losses.
Medicare ACOs run under three main programs:
To realize savings, an ACO needs to combine claims and clinical data through strong healthcare interoperability, find patients at increased risk, close care gaps, and measure itself against its benchmark. Population health management software is the tool that makes all of this possible.
How We Evaluated These Vendors
Each of these platforms was assessed according to six different aspects of an ACO, thereby providing a useful comparison of population health management software in the context of healthcare organizations.
LEAD runs from 2027 through 2036, making it the longest ACO model CMS has ever tested. It changes what your software must track, and for how long.
With more than 420 ACOs participating in MSSP and 103 ACOs in REACH, the software vendors for population health management need value-based care software that can support both programs.
Here are our top eight population health management software vendors for ACOs in 2027, compared by features, capabilities, and best fit.

1. Bacancy Technology
Bacancy Technology has built healthcare software since 2011, with 1,050+ engineers, 250+ healthcare specialists, and 200+ HIPAA-aligned platforms shipped. We build custom population health management software for ACOs, IDNs, payers, and value-based care organizations.
Key Features:
ACO & Value-Based Care Capabilities: We have population health management solution, which is compatible with MSSP, REACH ACO, and Commercial ACO. The tools in place include attribution tracking, benchmarking dashboards and reporting for HEDIS, MIPS, and eCQM. For our Regional MSSP ACO for 78,000 attributed lives, we recorded $9.3 million in Year 1, and we met 19 out of 22 quality measures.
Key Differentiator: You own the platform and its logic, so it can adapt to LEAD’s 10-year benchmark without waiting on a vendor’s roadmap. We deliver in two-week sprints with demo-driven reviews.
2. Innovaccer
Innovaccer is a healthcare AI company that serves providers, payers, the public sector, and life sciences organizations. By its own count, its platform manages 80M+ patient records across 130+ organizations, including six of the top 10 US health systems. Black Book ranked it #1 for PHM Data Integration and Analytics in both 2025 and 2026.
Key Features:
ACO & Value-Based Care Capabilities: In April 2026, CMS accepted Innovaccer, through Story Health Partners, into the first ACCESS model cohort. ACCESS patients stay attributed to the health system’s ACO, and program spend is excluded from ACO benchmarks for the first 18 months.
Key Differentiator: A program where Innovaccer is outcome responsible, ensuring that health systems do not have to create their own infrastructure.
3. Arcadia
Arcadia deals with healthcare data and analytics for providers, payers, accountable care organizations, life sciences, and government. Arcadia Ascent offers solutions in analytics, AI, and workflow management, while Arcadia CareJourney offers market intelligence services to networks and referrals.
Key Features:
ACO & Value-Based Care Capabilities: The population health analytics platform of Arcadia Health uses information from more than 40 EHR vendors and combines them in one complete view. It is NCQA HEDIS and Data Aggregator Validation Certified.
Key Differentiator: Care Journey acquired in 2024 to include benchmarking of 300M+ patients.
4. Lightbeam Health Solutions
Lightbeam is a Dallas-headquartered firm that offers an AI-driven population health platform along with advisory and care solutions. It caters to payers and providers who need claims, clinical, and social risk data to be converted into prioritized care workflows. The remote patient monitoring program offered by Lightbeam has been named Best in KLAS for two years consecutively with a score of 93.6.
Key Features:
ACO & Value-Based Care Capabilities: Lightbeam is tailored to MSSP participants and ACOs gearing up for LEAD. The NCQA gave pre-validation of the Lightbeam Platform for Population Health Management on December 10, 2025.
Key Differentiator: Device-less RPM assists those who lack ready access to their medical devices, hence helping many needy people.
5. Epic Healthy Planet
Healthy Planet is Epic’s population health module, built into an EHR that covers about 42% of the acute-care market. It combines lab results, risk scores, paid claims, and external system data so care teams can monitor patient health and act on it. Black Book ranked it #1 for Core EHR population health management.
Key Features:
ACO & Value-Based Care Capabilities: It provides for care coordination between providers and care teams as well as the tracking of quality metrics for value-based care initiatives.
Key Differentiator: Everything is included in the chart used by clinicians, and therefore there is no need for login.
6. Persivia CareSpace
Persivia offers CareSpace, a platform for population health, care coordination, and value-based care operations. Gartner Peer Insights lists it in the healthcare provider population health management market. Persivia has also published a detailed LEAD model whitepaper to help ACOs plan.
Key Features:
ACO & Value-Based Care Capabilities: CareSpace helps with billing and reimbursement under various payment models. Persivia has issued complete guidelines for the LEAD model in January 2026.
Key Differentiator: The care workflows and value-based care billing occur under one software application.
How to Choose the Right Platform for Your ACO
A compelling demonstration does not prove fit. Before signing, make sure you are able to assess each solution against your own programs and data. Here are some guidelines on selecting population health management solutions for ACOs:
The leading population health management vendors in the US each suit a different ACO. Test 2 or 3 population health management software vendors on your own CCLF data. If none fits, a healthcare software development partner can build the ACO management software and population health analytics software you need.
The system should be capable of monitoring a benchmark set over a period of ten years, expert-level events in CARA, and high-need patients in the core ACO. The CMS LEAD Model Webpage gives the whole framework. Ask for the demonstration of LEAD reporting currently in production by the population health management software vendors.
Buy when your platform supports your programs and EHR mix. Develop when you need data flows or workflows that even the best population health management platforms for accountable care organizations cannot support.
REACH for ACO will end on December 31, 2026. LEAD program will take its place from January 1, 2027 to the year 2036.
There must be included, at the very least, CMS claim files (CCLF or BCDA), clinical data from EHR, ADT alerts, and attribution files. Moreover, the refresh frequency of the data sources must also be ascertained.
MSSP is a permanent Medicare program established by the federal rulemaking, with over 420 ACOs participating. LEAD is a ten-year test model from the CMS Innovation Center that replaces ACO REACH.
Yes. The CMS developed LEAD in order to overcome the financial and administrative obstacles that had in the past kept small, independent, and rural physicians out of prior ACO models.
The ACCESS Program started in July 2026 and offers rewards for performance related to chronic disease management using technology. Participants in the ACCESS Program will still be accountable to their ACOs during the first 18 months.
NCQA evaluates population health tools against its accreditation standards. Organizations using prevalidated tools receive automatic credit towards NCQA surveys, thus reducing documentation burden.
