The Hidden Cost of Doing Nothing: A 2026 Wake-Up Call for Doctors

The Hidden Cost of Doing Nothing: A 2026 Wake-Up Call for Doctors

Ask five different practices what “MIPS services” means to them, and you’ll likely get five different answers. For one practice, it’s a piece of software that submits quality measures once a year.

For another, it’s a full team handling everything from measure selection to audit documentation. That inconsistency is exactly why so many practices end up disappointed with the support they’ve signed up for, they assume they were buying one thing and receiving another.

Heading into the 2026 performance year, with CMS holding the program’s core structure steady but continuing to refine measures underneath it, understanding what MIPS services should actually include has never mattered more.

What Falls Under “MIPS Services”?

At a basic level, MIPS registry refers to any external support that helps a clinician or practice participate in the Merit-based Incentive Payment System, one of the two tracks under Medicare’s Quality Payment Program. That can span a wide range of work, including:

  • Selecting quality, cost, and improvement activity measures appropriate to a specialty
  • Extracting and validating clinical data from an EHR
  • Submitting data to CMS through a Qualified Registry or QCDR
  • Modeling likely scores before the submission deadline
  • Advising on MIPS Value Pathway (MVP) participation
  • Maintaining documentation in case of a CMS audit

Some vendors offer only a slice of this list. Others build their entire business around covering all of it. Knowing which one you’re evaluating matters just as much as knowing whether their price is fair.

MIPS Reporting Services vs. Broader MIPS Support

It helps to separate two related but distinct ideas.

MIPS Reporting Services

This is the narrower, submission-focused piece, collecting data and transmitting it to CMS accurately and on time. Reporting services are essential, but on their own they’re reactive by nature: they process what they’re given rather than actively shaping your strategy throughout the year.

Broader MIPS Services

Broader services by MIPS Registry extend well beyond submission. They include the strategic work that happens before data ever reaches CMS, choosing measures that reflect your specialty well, monitoring performance gaps mid-year, and adjusting course while there’s still time to improve a score rather than simply reporting whatever numbers exist at year-end.

Practices that only need reporting services tend to already have strong internal data workflows. Practices without dedicated staff for quality reporting usually get more value from the fuller service model, even at a higher price point.

Why 2026 Is a Good Year to Reassess Your MIPS Support

CMS kept the four MIPS category weights unchanged for 2026, Quality at 30%, Cost at 30%, Promoting Interoperability at 25%, and Improvement Activities at 15%, which gives practices a stable framework to plan around. But stability at the category level doesn’t mean nothing has changed underneath it. Quality measures have again been added, revised, or retired, the Improvement Activities inventory has been refreshed, and CMS continues expanding MIPS Value Pathways as its preferred long-term reporting model.

The performance threshold remains at 75 points, and CMS has confirmed it will hold there through the 2028 performance year. That consistency actually raises the stakes: with the bar unchanged, the practices that pull ahead are the ones actively refining their measure mix and catching documentation gaps early, not the ones treating MIPS as a once-a-year formality.

What to Look for in a MIPS Services Provider

Verified CMS Status

Confirm the vendor is genuinely listed as a CMS-approved Qualified Registry or QCDR before handing over any data. This single detail determines whether your submission is even valid.

Specialty-Specific Measure Access

If your field has a QCDR offering specialty measures, common in areas like radiology, gastroenterology, and orthopedics, that access alone can meaningfully improve your Quality category score compared to relying on generic national measures.

Transparent, Ongoing Reporting

A good provider shows you where you stand mid-year, not just at submission time. If a measure is trending low in June, you want to know then, not in the final report.

All Four Categories Covered

Some providers lean heavily on Quality and treat Cost, Promoting Interoperability, and Improvement Activities as secondary. Since all four contribute to your final score, make sure your provider is actively managing each one.

Audit Support

CMS can request documentation supporting a MIPS submission well after the performance year closes. Ask upfront whether your provider retains source-level records and can produce them if needed.

Final Thoughts

The right MIPS services should feel less like a vendor you check in with once a year and more like a partner keeping an eye on your score alongside you. Whether you need full strategic support or focused MIPS reporting services to handle submission cleanly, the goal is the same, catching problems while they’re still fixable, not after the payment adjustment has already landed. Take the time to match the level of support to what your practice genuinely needs this year, and MIPS stops feeling like a compliance burden and starts working the way it’s supposed to: as a fair reflection of the quality care you’re already providing.
