CMMI announced new payment models in late 2025 targeting chronic disease (ACCESS, MAHA ELEVATE), drug pricing (GLOBE, GUARD, BALANCE), and accountable care (LEAD, replacing ACO REACH), with implications for hospitals, physicians, and ACOs. These models signal a broader shift toward mandatory episode-based payments, international drug pricing benchmarks, and technology-enabled chronic care in traditional Medicare.
