VHA Home HealthCare’s dedicated service providers partner with clients and their families to determine the best approach for providing care for each unique client situation. Virtual Care services are conveniently provided by telephone and/or by video either with a smartphone, tablet or computer and enable clients to work with their service providers remotely to reach their goals.
In many situations and depending on the needs and preferences of each client and family, VHA Virtual Care can be used in combination with in-person visits. VHA Home HealthCare provides a Welcome Kit to all of our new clients and families (Download PDF) to introduce our services and to provide information about our care.
The self-assessment is intended to help give you an idea of whether virtual care might be an appropriate way for you or your loved one to receive some health care services from VHA Home HealthCare. This assessment will take about 1-2 minutes to complete and consists of 5-6 questions (depending on your responses).
