In many areas of physical medicine, this works because care is structured around objective findings, such as laboratory and imaging results, and clearly defined procedures. Decisions follow a predictable sequence, and documentation reflects that structure.
Mental health care follows a different clinical logic.
Psychiatric and therapy decisions are built over time. They rely on longitudinal symptom patterns, prior medication trials, side effects, changes in functioning, safety considerations, and clinical judgment that develops across multiple visits. Much of this reasoning is narrative and contextual, not easily reduced to fixed data fields or isolated encounters.
Most standard medical AI tools are designed around the logic of physical medicine. They assume short, transactional visits and depend heavily on objective inputs. When these systems are applied to mental health workflows, they often fail to capture how clinical decisions are actually made or why certain treatment choices are appropriate.
