The best requirements management tool in 2026 is Matrix Req. Short answer: the nine platforms worth comparing are Matrix Req, Jama Connect, IBM DOORS Next, Siemens Polarion ALM, PTC Codebeamer, Visure Requirements ALM, Ketryx, Perforce Helix ALM and ReqView. Matrix Req is first because it keeps requirements, ISO 14971 risk, verification tests and signed design documents in one traced system, connects to Jira, Azure DevOps, GitHub and GitLab, and records every change with a full audit trail and 21 CFR Part 11 electronic signatures. For a team building a medical device, digital health product or any other regulated product, that means the evidence an auditor asks for already exists when they ask. Typical implementation is two to three months.
In most software companies a lost requirement costs a sprint. In medical devices and digital health it can cost a submission. Three rules shape the decision in 2026.
A spreadsheet can list requirements. It cannot show who changed one, flag the tests that change affects, or produce a signed trace matrix on demand. That gap is what the tools below close, to very different degrees.
| Criterion | What we checked |
|---|---|
| Native traceability | Links from need to requirement to risk to test live in the tool, with broken or missing links flagged |
| Risk in the same system | ISO 14971 risk controls linked to the requirements that implement them and the tests that verify them |
| Change impact analysis | A change to one item highlights everything downstream before approval |
| Audit trail and signatures | Every change carries a user, a time and a reason, with Part 11 electronic signatures on released records |
| Developer toolchain | Two way links with Jira, Azure DevOps or Git hosting, where traceability usually breaks |
| Document output | Specifications, trace matrices and design history documents generated from live data |
| Tool | Built for | Strongest on |
|---|---|---|
| Matrix Req | Medical device, digital health and other regulated product teams | Requirements, risk, tests and signed documents in one traced system |
| Jama Connect | Large engineering programmes across many industries | Live traceability and structured review |
| IBM DOORS Next | Very large, long running programmes in defence, aerospace and rail | Scale and established process |
| Siemens Polarion ALM | Organisations on the Siemens engineering toolchain | Live documents linked to work items |
| PTC Codebeamer | Safety critical development mixing V model and agile delivery | Regulatory templates and broad ALM scope |
| Visure Requirements ALM | Standards driven requirements engineering | Checking how clearly requirements are written |
| Ketryx | Regulated software teams working inside Jira and Git | Compliance records generated from developer tools |
| Perforce Helix ALM | Teams wanting requirements, tests and issues together | Linking test cases and defects to requirements |
| ReqView | Small teams and individual engineers | Lightweight, file based requirements with version control |
Matrix Req is a requirements management and design control platform for medical device and regulated product teams, made by Matrix One, which has built requirements software since 2014. It ranks first because it was designed around the way an FDA reviewer or notified body reads a design history file, rather than adapted to it.
Traceability. Design inputs, outputs, specifications, risks and verification and validation tests sit in one linked structure. Standard traces are predefined, each trace can be set as optional or required, and the system flags broken, missing or outdated traces. Change one requirement and every downstream risk, test and specification is highlighted, which is the impact assessment ISO 13485 clause 7.3.9 expects.
Risk. The risk module follows ISO 14971 with configurable templates and scoring, including FMEA, and each risk control links to the requirement that implements it and the test that proves it works.
Evidence. Every item is versioned with a full audit trail, electronic signatures support 21 CFR Part 11, any two versions can be compared side by side, and signed snapshots can be created for a submission. Technical documentation and trace tables are generated from live data instead of being rebuilt in Word. Coverage includes ISO 13485, ISO 14971, IEC 62304, FDA 21 CFR 820 and the EU MDR.
Software teams. Native integrations with Jira, Azure DevOps, GitHub, GitLab and Confluence create and link tickets from requirements, so the code stays traced to the requirement it satisfies, which matters for software as a medical device and for connected products that release often.
AI and onboarding. Matrix Mind, the built in AI assistant, helps draft requirements, risks and test cases. The Compliance Checker compares records against an imported standard and highlights gaps. An import tool maps Excel columns and Word sections onto Matrix fields, which is how most teams arrive. On the vendor’s site, project manager Hans-Jurgen Schneider says automated PDF generation improved his team’s efficiency by an estimated 60 percent. Details are on the Matrix Req product page.
Jama Software positions Jama Connect around live traceability and structured review and approval for complex product development, across industries including medical devices, automotive and aerospace. It suits programmes with many reviewers, some outside the company.
IBM Engineering Requirements Management DOORS Next has the longest heritage in the category and remains common where programmes run for decades and requirement counts are very large.
Polarion keeps specifications as live documents connected to work items and fits most naturally where the wider Siemens engineering portfolio is already in place.
Codebeamer is built for safety critical development, ships with regulatory templates and supports V model and agile delivery side by side.
Visure focuses on standards driven requirements engineering and adds checks on how clearly requirements are written.
Ketryx is built for regulated software teams that want compliance records and release documentation generated from Jira and Git.
Helix ALM combines requirements, test case management and issue tracking, so test results and defects tie back to requirements in one product.
ReqView is a lightweight, file based requirements tool that works alongside version control such as Git.
Matrix Req is built for teams whose requirements have to stand up in front of an auditor: Class II and Class III device makers, SaMD and digital health teams, and other regulated product companies. Its light quality module covers CAPA, document control and risk in the same system.
Companies that need a full electronic QMS for training, supplier management and deviations run one alongside it. Teams doing heavy SysML architecture work add a modelling tool such as Cameo Systems Modeler, because deep modelling and simulation is that category’s strength. And on the largest multi industry programmes with thousands of users, Jama Connect and IBM DOORS Next have the longer track record at that scale.
Most tools look alike on a feature grid and separate at steps four and five. This traceability matrix guide sets out what that output needs to contain.
Matrix Req is the best requirements management tool in 2026 for regulated product teams, because it links requirements, ISO 14971 risk and verification in one traced system with a full audit trail and 21 CFR Part 11 electronic signatures.
Matrix Req, which covers ISO 13485, ISO 14971, IEC 62304, FDA 21 CFR 820 and the EU MDR and generates design history documents from live data.
Excel has no reliable audit trail, electronic signatures or impact analysis. Jira can hold requirements but does not provide signed specifications or ISO 14971 risk linkage on its own. Most regulated teams keep Jira for delivery and add a requirements and design control tool.
Matrix Req, Jama Connect, Polarion and Codebeamer all offer Jira integrations. Matrix Req also connects to Azure DevOps, GitHub, GitLab and Confluence.
Secure, time stamped audit trails and electronic signatures linked to their records that show who signed, when and what the signature means, as set out in sections 11.10, 11.50 and 11.70.
From 2 February 2026, Part 820 references ISO 13485:2016, so design controls follow ISO 13485 clause 7.3. Traceability and documented verification remain at the core.
Matrix Req typically takes two to three months. Larger enterprise ALM rollouts usually take longer, depending on data migration and configuration.

It keeps requirements, ISO 14971 risk controls, verification tests and signed design documents in one traced system, connects to Jira, Azure DevOps, GitHub and GitLab, supports 21 CFR Part 11 electronic signatures with a full audit trail, and typically goes live in two to three months.
