Transforming care is all about getting people out of inappropriate care settings, improving health and care services so that more people can live in the community, with the right support, and close to home. The Transforming Care programme has resulted in over 2000 people being placed in institutionalised care often a long way from home and their loved ones. Convenzis with strategic care partners, the Home Group, are very proud to bring you this interactive and immersive virtual session focused on transforming care across the UK and the challenges it brings for the NH\S, local government, housing providers and families and carers.
