Remote patient monitoring (RPM) has shown promise in aiding safe and efficient remote care for chronic conditions; however, its use remains more limited within the hospital at home (HaH) model of care despite a significant opportunity to increase patient eligibility, improve safety, and decrease costs.
Expanding remote patient monitoring (RPM) within Hospital-at-Home programs could safely increase eligibility for home-based acute care, improve patient safety, and lower costs by enabling more frequent vital sign and ECG monitoring outside the hospital. Integrating RPM effectively requires robust technology, clear clinical guidelines, and supportive policy frameworks to maximize benefits and scale home-based care delivery.
