Simplified “No Wrong Door” care coordination systems can help older adults and caregivers access long-term care services more efficiently while reducing confusion during critical healthcare transitions. Better integration of community support, Medicaid resources, and care navigation programs may improve aging-in-place outcomes and reduce unnecessary hospitalizations in the U.S. healthcare system.
