Frequent hospital users (FHUs) show high risk of clinical instability. A hospital-at-home (HaH) service (“GIROT”) was created in Florence, Italy, to reduce hospitalizations in older adults vulnerable to hospital-related complications.
Hospital-at-home care for older adults who frequently use hospitals can reduce the burden of repeated inpatient stays—improving patient comfort and potentially lowering healthcare costs. Successfully expanding these programs requires coordinated care models tailored to complex needs and support systems for high-need elderly populations.
