The Individually Tailored monitoring at home after HEART failure (IT-HEART) trial evaluates whether a non-invasive multi-component telemonitoring program improves outcomes in HF patients after HF-hospitalization.
Personalized telemonitoring programs for heart failure patients focus on education, self-care support, and continuous home monitoring, addressing major gaps like low health literacy and poor self-management after hospital discharge.
