Medication reconciliation or med reconciliation is the formal process of ensuring that the medication list of patients admitted in hospitals is as up-to-date as possible. The medication list contains drug name, drug dosage, drug frequency, and route of administration. The medicine reconciliation process is based on five critical steps, including – Development of a current list of medications, developing a list of prescribed medications, comparing the medicines mentioned in these two lists, making clinical decisions based on this comparison, and providing the newly updated list to the caregivers or patients.
