Population Health Management can be defined as the collection of clinical data of patients through multiple health information technology resources, followed by the analysis of that data into a single, actionable patient record, and the actions through which health care providers can develop both clinical and financial outcomes. Population Health Management solutions help in analytics and care management functions which in turn assist in understanding, analyzing, and proactively managing patient populations. Population Health facilitate the exchange of patient information for analyzing and assessing a patient’s acuity and risk of hospital admission besides the financial risk associated with ongoing payer contracts. Population health solutions include services that are mostly centered around shifting payment models and value based care.

The Atlas compares value-based coverage models against purely fee-for-service plans and networks.
